Please read and accept the following terms prior to completing the submission for your application

Certification of Information: I am applying for employment with PeopleLink Staffing and upon my agreement below, I certify that the information I have provided in the application process is accurate, truthful and complete to the best of my knowledge. I understand and agree that any FALSE, incomplete or misrepresentation of information discovered before or after a job offer will result in rejection of my application or dismissal at any time during my employment.

Application and Equal Opportunity: This application is not an employment contract, job offer or promise of employment. It is intended to evaluate suitability for employment. Verification and Background Investigation: PeopleLink Staffing may, upon execution of this authorization, investigate information contained in my employment application, social security number verification and/or other background information, the results of which will be used as one factor in making employment decisions. In the event that information from certain consumer reports is utilized in whole or in part in making an adverse employment decision, before making the decision, PeopleLink Staffing will provide you with a copy of the consumer report as required by the Fair Credit Reporting Act.

Release of Information: My agreement below authorizes and gives full permission to have PeopleLink Staffing examine any and all consumer reports, investigative reports, social security number verification and/or criminal records and arrests on file at any time after the receipt of this authorization and throughout my employment (if any). I hereby release PeopleLink Staffing, its agents, and any persons, employers, companies, clients, agencies and authorities who verify or provide information regarding my references or background from any liability for any damage whatsoever for disclosing or issuing any such information.

Drug Use and Testing: PeopleLink Staffing has a long-standing commitment to its employees and customers to promote a safe and healthy work environment. Therefore PeopleLink Staffing has incorporated a “Zero Tolerance” drug and alcohol policy. I understand that PeopleLink Staffing prohibits the use of illegal drugs. My agreement below certifies that I am willing to provide a urine, blood, hair or saliva specimen for drug and/or alcohol testing prior to and/or during my employment as a condition of my employment, or if there is any reasonable suspicion of drug or alcohol use. I understand that the company policy requires a drug and alcohol test any time there is an on-the-job or near-miss accident. I hereby authorize and consent to all such tests and acknowledge my understanding that positive drug/alcohol test or refusal to submit to a required drug/alcohol test will result in my dismissal. I hereby release PeopleLink Staffing, its clients and any clinic, individual or test product manufacturer that may administer or provide a drug or alcohol test from any and all claims arising from the results of such a test, and from any action taken on the basis of those results.
